To receive images using your form:
Mouse over your form, click Options then Edit
From the Add Field tab, drag and drop the File Upload field to your form design area.
Click on the File Upload field which you added to show the Field Settings.
Configure the field to allow the upload of images. Check off the formats for images such as png, jpg, jpeg, gif.
There are also other formats readily available for your File Upload button.
